Abstract

Communities are facing issues in accessing basic healthcare services due to lack of collaboration in the improvement of accessing the healthcare services. This papers aims to provide the healthcare practitioners and the community with an opportunity to enhance collaboration in the community to improve the health of the community to eliminate disparities in access of basic healthcare services for communities and in order to enhance quality of healthcare service in community collaboration and continuous planning.

In order to eliminate disparities in access of basic healthcare services for communities and in order to enhance quality of healthcare service in community collaboration and continuous planning is required in understanding of society, population and environment. Health plans for community should be flexible and fluid, acting in response to changes in social demographics, dynamic patterns of illness and health, and revolutionary progresses in technology and science.

Academic centers and Foundations are recognizing successful community policies, examining the top secrets of their accomplishments, and broadcasting them as efficient approaches and models (Hurley et. al, 2007).
